Webinar ID: # 1035
Recorded Webinar @ All Day
Duration: 60 minutes
Description:
Discharge planning in hospitals must comply with the regulations set forth by the Centers for Medicare and Medicaid Services (CMS) and Conditions of Participation (CoPs). These regulations aim to ensure patients' safe and effective transition from the hospital to their next level of care, whether it's home, a skilled nursing facility, or a healthcare setting. Every hospital that accepts Medicare and Medicaid must comply with the CMS discharge planning guidelines. These standards must be followed for all patients, not just Medicare or Medicaid.
Discharge Planning is a crucial process in healthcare that involves preparing a patient to leave a healthcare facility, such as a hospital or rehabilitation center, and ensuring a smooth transition to the next level of care or home. Discharge planning isn't just about the paperwork; it's about ensuring patients receive the care they need once they leave the hospital.
The webinar starts by looking at the old rules, and then we'll talk about the latest updates in the Medicare program and how they will affect the jobs of RN case managers and social workers. You'll find out how to make sure your procedures take into account the challenges of the new healthcare system. You'll also learn how to ensure your roles as a case manager or social worker are ready for the upcoming changes and follow the federal rules correctly.
